Spencer's Results

Dear Mr. and Mrs. Farnum,

Spencer's results came back as intermediate to blastic (or large cell) lymphoma this time.  This is what I expected.  This form of lymphoma is more aggressive than the small cell form he started out with, but is sometimes more responsive to chemotherapy.  There is also a good oral option called Lomustine or CCNU.  This drug causes minimal stomach upset, but can lower his white blood cell count far more than the chlorambucil.  It is given once a month, with CBC checks every two weeks.  We should recheck his ultrasound in 1 month before giving his next dose to make sure it is working, because if it is not, we will likely have to go to an injectable chemo or decide to stop. If his twitching is due to spread to the brain, this drug does penetrate into the brain as well.

We need to have the dose of Lomustine compounded to a size that is appropriate for Spencer.  Cross River pharmacy will do this for us.  You just need to call Michelle at our office 914-241-7700 to have her call in the prescription.  You can come up to Cross River to pick it up or they can mail it to you.  I would recommend giving this one capsule like you give the Hepatosupport, since it is essential that he receive the entire dose.  You can call Cross River at 914-763-3152 to arrange for pick up or delivery after Michelle has called in the prescription.

Continue the prednisone & Hepatosupport at the current doses.  You can decrease the Chlorambucil back to every 3rd day once you give the Lomustine.  Schedule a recheck appointment at 2 and 4 weeks after the dose.

I wish I had better news, but I am glad that we have some treatment options that will be minimally stressful for our boy.  Please let me know what questions you have.

Hi, Dr. Fryer!

I just thought I'd drop you a line to give you the Spencie update.

He's doing well on a lot of fronts - he hasn't thrown up at all since Thursday, when we brought him in, and we haven't had to use the nausea medicine at all.  He's still eating fine, and he's taking the Prednisone, Chlorambucil and Hepato support without any trouble - we decided to suspend the Denosyl for the time being, just so he won't become suspicious of the pill pockets we're using - we don't want to overdo it with those - if he figures it out again that we're hiding pills in those things, we're in big trouble!  Switching flavors did seem to do the trick - for the time being.  So we're trying to keep the pills at the minimum that we can get away with for now.  Does that sound okay?

One thing concerning us is that, although he had a great rebound over the week-end and seemed back to normal, he now seems to be becoming a little more low-energy again.  Not really weak per se, just lethargic and lazy.  He did attempt to play a little bit tonight, which made me happy, but he seemed to be a little uncoordinated in his back end.  Again, it didn't seem as though he was weak, but more just a little out of control of his hind quarters.  But - he's still able to jump up on stuff without any trouble.  So it's a little puzzling, and it's concerning us a little.

I also noticed that he had a huge amount of grumbling and gas sounds coming from his little belly this morning - I remember also noticing that happening to him several months ago when he was becoming ill.  This morning, I would have thought he was just hungry, but he wasn't begging for food at the time when I heard this - as a matter of fact, he was sleeping with his belly against my ear - it was so loud it woke me up.  Seemed  a bit odd....

Other than that, we're going day to day and trying to keep good thoughts going.  We'll bring him in on Friday a.m. as we had discussed, but if you have any advice or thoughts in the meantime, we'd sure welcome it/them.  Thanks a lot!  Hope you had fun at the wedding last week-end.
